$50.00 PER SQUARE​

Please fill out the FORM below after viewing the grid to see what squares are available.

(1) Enter your RX Poster Name. This will be displayed on the grid.
(2) Select the squares you wish to purchase by ticking the appropriate checkboxes.
(3) Select your means of payment.
(4) Enter the total amount of your payment.
(5) Fill in either Email Address or Mailing Address depending on means of payment.
(6) Finally, enter your FULL real name.

All money will be sent to Betallsports by Pay Pal (New Address) or Money Order.
Squares available on a first come, first serve basis.
Could be up to 20 winners depending on where the winning numbers land on the grid.

I just put Betallsports for the name and USA for country, no problems on my end.
