Pressure makes people do things they don't normally do. Lopez usually very good but he couldn't throw a strike. Rizzo bailed him out on the second strike, that was a ball too. He was feeling the pressure to make a play, gripped too hard and yanked it. They are humans, not machines. Sometimes people forget that.
