Firepay works great. I've funded my firepay account for various amounts and the fee is always $1.50. I'm not sure what the neteller fees are, but I think $1.50 is reasonable enough. And i've never been charged by a book or poker site using Firepay. Of course you still have the same set up time to get your account acctivated.
mvbski - I have found more sites take NETeller and NETeller has a ATM card you can use at most ATM machines. But I hear Firepay take more CC's than NETeller, not sure how they do that though.
There are many books that have complained about CHARGEbacks from users that use Firepay so if that's true it's not cool and I would have to support the sites I do business with more than I would support a payment solution like Firepay that let's that happen to the sites.

EverFresh - You're right. My bad. I was thinking of PrePaidATM. They seems to process more CC's than NETeller.

T.O.S says governed by the laws of Texas, so they're in Texas, or the site from which they copied and pasted the T.O.S. page is.
The "example" site of a BridgeTrust merchant is in fact a Best Buy affiliate page linked to its own domain name and run off of the same server as the BridgeTrust site (68.208.208.xx) Both domains traceroute to BellSouth.net-owned servers located in the Atlanta area.
While this might be a legit service, it is wholly unsuitable for books. Being U.S. based they will be subject to the exact same regulations as any other U.S. company with regards to processing credit card transactions for the purpose of funding gambling accounts. Further, their chargeback policy is weak -- I don't understand why books deal with any company that allows chargebacks to occur at all.

One new company (in Business 1 1/2 years) I am keeping my eye on is Moneybookers which is located in the UK.
They Indemnify books from charge backs and their fees are low.
