The crash between the USS Fitzgerald and a Philippine cargo ship off the coast of Japan was likely caused by the Navy crew members' failure to respond in time.
The crew on Fitzgerald 'did nothing until the last second', according to one official familiar with the investigation into the June 17 crash off the coast of Honshu, Japan, that claimed the lives of seven American sailors, CNN reported.
Preliminary findings in the inquiry indicate crew members on the Fitzgerald failed to acknowledge the ACX Crystal was approaching and did not the necessary action to avoid a collision, according to two Defense Department officials who spoke to CNN.

According to Navy spokesman Commander Ron Flanders, the accident occurred at 2.20am, although the Japanese assert the crash took place at 1.30am.
Coast guard official Tetsuya Tanaka said last month that authorities were trying resolve what happened during the disputed 50 or so minutes.
The cargo ship's bow, which protrudes underneath the water, punctured the steel armor of Fitgerald.

CNN reported five of the seven soldiers killed in the accident were almost instantly 'incapacitated', and died quickly as a result.
The other two likely lived much longer and attempted to escape the flooding, but an official told the network 'the ship somehow lost communication' with the duo.
Emergency hatches were closed on the compromised berthing compartments to stop the ship from sinking.
'Heroic efforts prevented the flooding from catastrophically spreading which could have caused the ship to founder or sink,' said Vice Admiral Aucoin at a press conference the day after the collision.
The seven who died are: Dakota Kyle Rigsby, 19; Carlos Victor Ganzon Sibayan, 23; Xavier Alex Martin, 24; Shingo Alexander Douglass, 25; Rehm Jr, 37; Ngoc T Truong Huynh, 25; and Noe Hernandez, 26.
Captain Benson was airlifted to hospital after the accident, and was reportedly in stable condition on Saturday. In total three sailors were injured; all have since been released from a Navy hospital.

Again, how could this possibly happen? The Captain was asleep. Was the crew getting high? They had to be fuckin off. Now 7 soldiers are dead. Heads gotta roll.

if they reported anything accurately they would not be CNN.
