Have any of you LVH players ever had a proxy make a mistake on your entry?

I would have had a 4-1 week but due to Proxy entering in a pick I did not ask for, I went 3-2

Proxy has not gotten back to me. I discovered it during the game as I happened to check other player selections. I rather not mention proxy name, but this ought to be an infallible process.
