The source I got that question from claims that it actually happened, but
didn't provide the details. Let me reword it this way:

4. How can a batter hit into a triple play without any fielder touching
the ball?

Hey hey...after a moment of honest thought, I know at least one answer to #4

**Bases loaded, no outs of course. Batter strikes out on a passed ball or wild pitch that the catcher does not touch.

With first base occupied, he cannot advance...however it's a live ball.....the runners break and then the two trail runners are somehow foolish enough to pass the runner ahead of them on the basepaths and they are each called out.
